The Meadowview Elementary School Health Office is staffed by building nurse Jill Hauge, RN.
Services that are provided in the Health Office may include but are not limited to are:
1. Giving prescribed daily medication and other "as needed" medications ( MD and Parent/Guardian written permission is required). This requirement includes over the counter medications such as Tylenol and Ibuprofen, etc.
2. Providing evaluation and/or care as needed for students with illnesses or injuries.
3. Assisting with collection of data and record keeping of immunizations and various health screenings ( i.e. vision and hearing screening).
Helpful Reminders:
If you are calling in an absence for your child, please call 651-460-3105.
If your child needs to go home due to illness or injury, you or your designee must come to the Health Office to pick up your child and sign him or her out in the main office.
If you anticipate that your child may need medication such as Tylenol or Ibuprofen during the school day for headaches, colds, braces tightening, etc., please complete the Medication Authorization Form located on the District Health Services page. Also, please bring in a small bottle of the medication you want on hand for your child in the original bottle, labeled with your child's name. No baggies of medication will be accepted.
